Abstract

Epilepsy is a common neurological disease that can affect all ages. Although the majority of people with epilepsy can have excellent seizure control with medication, about 30% will fail anti-epileptic drugs. For those with medically intractable epilepsy, recurrent seizures lead to elevated mortality and injury risks, and the seizures themselves are socially disabling. Fortunately, for many people with intractable epilepsy, epilepsy can be cured or seizures better controlled with surgery.Localization of the seizure focus followed by surgical resection provide the best opportunities to cure epilepsy. Having a better understanding of the neuro-anatomy and physiology of epilepsy improves our ability to define the epileptic network and effectively treat epilepsy. Minimal access surgical approaches result in more rapid recovery from surgery, less pain, and more satisfied patients.For individuals without an opportunity to cure their epilepsy, new and emerging technologies can improve their quality of life by preventing injuries and reducing seizure frequency and severity.This Special Issue will highlight advances in the diagnosis and surgical treatment of epilepsy, including imaging and electroencephalography (EEG) approaches to defining the epileptic focus, understanding the epileptic network, and characterizing the anatomical substrate of epilepsy.

Abstract

The work introduces a method for diagnosing design flaws in object oriented systems, and finding meaningful refactorings to remove such flaws. The method is based on pairing up a structural pattern that is considered pathological (e.g. a code smell or anti-pattern) with a so called design context. The design context describes the design semantics associated to the pathological structure, and the desired strategic closure for that fragment. The process is tool supported and largely automated.

Abstract

Abstract

Mitochondrial disease constitutes a complex and heterogeneous group of disorders resulting from a defect in mitochondrial respiratory chain (MRC) enzyme activity. In view of the dual regulation of the MRC, exercised by both the mitochondrial and nuclear genome, mutations in either mitochondrial or nuclear DNA can result in a MRC deficiency. Whilst a single organ can be affected, MRC disorders often result in a multi-organ system presentation with prominent neurological and myopathic features. The diagnosis of MRC disorders can be complex, and requires a coordinated interplay of a number of disciplines. However, biochemical determination of metabolites in blood, cerebral spinal fluid (CSF) and/or urine are generally considered to be first-line investigations for the diagnosis of these disorders, although they lack sensitivity and specificity. Furthermore, there is a lack of consensus on the overall utility of monitoring other biochemical parameters, which may be of diagnostic value. For example, although oxidative stress may contribute to the pathogenesis of mitochondrial disorders, few centers monitor this as part of their diagnostic repertoire. Therefore, the purpose of this Special Issue was to highlight potential biomarkers of mitochondrial disease and to discuss the appropriateness of biochemical markers to monitor disease progression and therapeutic intervention.

Abstract

MicroRNAs (miRNAs) constitute a large family of small, approximately 20–22-nucleotide, non-coding RNAs that are involved in gene regulation, mainly at the post-transcriptional level. Multiple lines of evidence have indicated that miRNAs play important roles in the maintenance of biological homeostasis and that aberrant expression levels of miRNAs are involved in the onset of many diseases, including cancer. In various types of cancer, miRNAs play important roles in tumor initiation and development. Recently, miRNAs have been demonstrated to also be secreted via small endosome-derived vesicles called exosomes—which are derived from multiple cell types—including immunocytes and cancer cells. Exosomal miRNAs exert important functions in cell-to-cell communication and have been investigated as prognostic and diagnostic biomarkers.

Neutrosophic logic and set are gaining significant attention in solving many real-life problems that involve uncertainty, impreciseness, vagueness, incompleteness, inconsistency, and indeterminacy. Abstract

There has been an observed decrease in the global mortality from cancer, mostly atributable to improved, particularly early, detection and prevention. For many carcinomas and leukaemias in adults, once the disease has reached a certain stage there are no therapies that are able to erradicate the cancer cells and cure patients. There has been progress in the treatment of acute myeloid leukaemia (AML) and remissions are achievable; however, the presence of chemoresistent blast cells leads to most patients relapsing, and relapse is difficult to treat and thus patients die due to their disease. Targeting these resistent cells and the leukaemia stem cells, which sustain the leukaemia, is crucial for an effective therapy for AML. Moreover, an increasing number of diverse mutations have been described in AML cells that disrupt the ability of these cells to undergo differentiation. The use of pro-differentiating agents to drive the blast cells to mature, and subsequently undergo apoptosis, provides another approach to therapy. Differentiation therapy, using all-trans retinoic acid (ATRA), an inducer of granulocyte differentiation, has been highly successful in the case of acute promyeloicytic leukaemia, a sub-type of AML, turning this disease into a curable malignancy.

Abstract

In the post-genomic era, many efforts have been devoted to better understanding the biological information encoded by the cell “glycome” in normal and pathologic conditions. The glycan signature of human cells plays a pivotal role in regulating fundamental biological processes, which are critical for cell physiology and for cancer as well.Galectins (also worded S-type lectins) are an evolutionarily conserved family of endogenous lectins, which bind carbohydrates with high specificity. These molecules, which can be found both intracellularly and in the extracellular milieu, are functionally active in converting glycan-containing information into cell biological programs. This fashionable mechanism of signal transduction plays a relevant role in regulating several biological functions, including RNA splicing, gene transcription, cell migration and differentiation, apoptosis, immune response, and tumor growth and progression.It is not surprising, indeed, that a large number of studies on galectin–glycan interactions and galectins expression and function in human diseases have been published in the recent literature, spanning from immunology to cardiovascular medicine, from diagnostic Pathology to nuclear medicine.The aim of this Special Issue of IJMS is to collect selected contributions in the field reporting data, concepts, and new ideas, which have the potential to be translated in a clinical setting in the near future, in order to improve the diagnosis and treatment of cancer and other relevant human diseases.
