Abstract

Diagnostic procedures are emblematic of medical work. Scholars in the field of social studies of medicine identify diverse dimensions of diagnosis that point to controversies, processual qualities and contested evidence. In this anthology, diagnostic fluidity is seen to permeate diagnostic work in a wide range of contexts, from medical interactions in the clinic, domestic settings and other relations of affective work, to organizational structures, and in historical developments. The contributors demonstrate, each in their own way, how different agents ‘do diagnosis’, highlighting the multi-faceted elements of uncertainty and mutability integral to diagnostic work. At the same time, the contributors also show how in ‘doing diagnosis’ enactments of subjectivities, representations of cultural imaginaries, bodily processes, and socio-cultural changes contribute to configuring diagnostic fluidity in significant ways.

Abstract

Mitochondrial disease constitutes a complex and heterogeneous group of disorders resulting from a defect in mitochondrial respiratory chain (MRC) enzyme activity. In view of the dual regulation of the MRC, exercised by both the mitochondrial and nuclear genome, mutations in either mitochondrial or nuclear DNA can result in a MRC deficiency. Whilst a single organ can be affected, MRC disorders often result in a multi-organ system presentation with prominent neurological and myopathic features. The diagnosis of MRC disorders can be complex, and requires a coordinated interplay of a number of disciplines. However, biochemical determination of metabolites in blood, cerebral spinal fluid (CSF) and/or urine are generally considered to be first-line investigations for the diagnosis of these disorders, although they lack sensitivity and specificity. Furthermore, there is a lack of consensus on the overall utility of monitoring other biochemical parameters, which may be of diagnostic value. For example, although oxidative stress may contribute to the pathogenesis of mitochondrial disorders, few centers monitor this as part of their diagnostic repertoire. Therefore, the purpose of this Special Issue was to highlight potential biomarkers of mitochondrial disease and to discuss the appropriateness of biochemical markers to monitor disease progression and therapeutic intervention.

Abstract

MicroRNAs (miRNAs) constitute a large family of small, approximately 20–22-nucleotide, non-coding RNAs that are involved in gene regulation, mainly at the post-transcriptional level. Multiple lines of evidence have indicated that miRNAs play important roles in the maintenance of biological homeostasis and that aberrant expression levels of miRNAs are involved in the onset of many diseases, including cancer. In various types of cancer, miRNAs play important roles in tumor initiation and development. Recently, miRNAs have been demonstrated to also be secreted via small endosome-derived vesicles called exosomes—which are derived from multiple cell types—including immunocytes and cancer cells. Exosomal miRNAs exert important functions in cell-to-cell communication and have been investigated as prognostic and diagnostic biomarkers.

Abstract

The proposed approach systematically searches for root causes of detected performance problems by executing series of systematic performance tests. The presented approach is evaluated by various case studies showing that the presented approach is applicable to a wide range of contexts.

Abstract

There has been an observed decrease in the global mortality from cancer, mostly atributable to improved, particularly early, detection and prevention. For many carcinomas and leukaemias in adults, once the disease has reached a certain stage there are no therapies that are able to erradicate the cancer cells and cure patients. There has been progress in the treatment of acute myeloid leukaemia (AML) and remissions are achievable; however, the presence of chemoresistent blast cells leads to most patients relapsing, and relapse is difficult to treat and thus patients die due to their disease. Targeting these resistent cells and the leukaemia stem cells, which sustain the leukaemia, is crucial for an effective therapy for AML. Moreover, an increasing number of diverse mutations have been described in AML cells that disrupt the ability of these cells to undergo differentiation. The use of pro-differentiating agents to drive the blast cells to mature, and subsequently undergo apoptosis, provides another approach to therapy. Differentiation therapy, using all-trans retinoic acid (ATRA), an inducer of granulocyte differentiation, has been highly successful in the case of acute promyeloicytic leukaemia, a sub-type of AML, turning this disease into a curable malignancy.

Abstract

In the post-genomic era, many efforts have been devoted to better understanding the biological information encoded by the cell “glycome” in normal and pathologic conditions. The glycan signature of human cells plays a pivotal role in regulating fundamental biological processes, which are critical for cell physiology and for cancer as well.Galectins (also worded S-type lectins) are an evolutionarily conserved family of endogenous lectins, which bind carbohydrates with high specificity. These molecules, which can be found both intracellularly and in the extracellular milieu, are functionally active in converting glycan-containing information into cell biological programs. This fashionable mechanism of signal transduction plays a relevant role in regulating several biological functions, including RNA splicing, gene transcription, cell migration and differentiation, apoptosis, immune response, and tumor growth and progression.It is not surprising, indeed, that a large number of studies on galectin–glycan interactions and galectins expression and function in human diseases have been published in the recent literature, spanning from immunology to cardiovascular medicine, from diagnostic Pathology to nuclear medicine.The aim of this Special Issue of IJMS is to collect selected contributions in the field reporting data, concepts, and new ideas, which have the potential to be translated in a clinical setting in the near future, in order to improve the diagnosis and treatment of cancer and other relevant human diseases.

Abstract

Although mortality rates have declined in recent years, the majority of cancers are still difficult to treat and the medical need for better cancer treatment is evident. The current anticancer armamentarium includes many active agents that are applied across tumor types. However, most of these broadly-active anticancer drugs have a small therapeutic index and barely discriminate between malignant and normal cells. In recent years the focus has shifted to the development of rationally designed, molecularly-targeted therapy for the treatment of a specific cancer, therefore offering the promise of greater specificity coupled with reduced systemic toxicity. NF-kB transcription factor family as emerged as such a promising target for cancer therapy. This Special Issue will explore the routes from NF-kB basic research, cancer research and oncogenomics into the development of NF-kB-based cancer therapeutics and biomarkers.We invite research and review papers in any area of the NF-kB field that are related, but not limited to, fundamental understanding of NF-kB signaling pathways, gene expression profiling, epigenetic regulation, diagnostic, prognostic and pharmacogenomic biomarkers, molecular targets driving the progression of human cancers, cancer drug development on these targets, clinical trial with new agents, and validation in animal models.We hope that this Special Issue reflects the exciting era that we are living in with respect to the field of NF-kB and its applications in cancer research.
