Abstract

Music is a complex, dynamic stimulus with an un-paralleled ability to stimulate a global network of neural activity involved in attention, emotion, memory, communication, motor co-ordination and cognition. As such, it provides neuroscience with a highly effective tool to develop our understanding of brain function, connectivity and plasticity. Increasingly sophisticated neuroimaging technologies have enabled the expanding field of music neuroscience to reveal how musical experience, perception and cognition may support neuroplasticity, with important implications for the rehabilitation and assessment of those with acquired brain injuries and neurodegenerative conditions. Other studies have indicated the potential for music to support arousal, attention and emotional regulation, suggesting therapeutic applications for conditions including ADHD, PTSD, autism, learning disorders and mood disorders. In common with neuroscience, the music therapy profession has advanced significantly in the past 20 years. Various interventions designed to address functional deficits and health care needs have been developed, alongside standardised behavioural assessments. Historically, music therapy has drawn its evidence base from a number of contrasting theoretical frameworks. Clinicians are now turning to neuroscience, which offers a unifying knowledge base and frame of reference to understand and measure therapeutic interventions from a biomedical perspective. Conversely, neuroscience is becoming more enriched by learning about the neural effects of ‘real world’ clinical applications in music therapy. While neuroscientific imaging methods may provide biomarking evidence for the efficacy of music therapy interventions it also offers important tools to describe time-locked interactive therapy processes and feeds into the emerging field of social neuroscience. Music therapy is bound to the process of creating and experiencing music together in improvisation, listening and reflection. Thus the situated cognition and experience of music developing over time and in differing contexts is of interest in time series data. We encouraged researchers to submit papers illustrating the mutual benefits of dialogue between music therapy and other disciplines important to this field, particularly neuroscience, neurophysiology, and neuropsychology. The current eBook consists of the peer reviewed responses to our call for papers.

Abstract

The coupling of several areas of the medical field with recent advances in robotic systems has seen a paradigm shift in our approach to selected sectors of medical care, especially over the last decade. Rehabilitation medicine is one such area. The development of advanced robotic systems has ushered with it an exponential number of trials and experiments aimed at optimising restoration of quality of life to those who are physically debilitated. Despite these developments, there remains a paucity in the presentation of these advances in the form of a comprehensive tool. This book was written to present the most recent advances in rehabilitation robotics known to date from the perspective of some of the leading experts in the field and presents an interesting array of developments put into 33 comprehensive chapters. The chapters are presented in a way that the reader will get a seamless impression of the current concepts of optimal modes of both experimental and ap- plicable roles of robotic devices.

Abstract

Cerebral palsy is a common pediatric problem and is the leading cause of childhood disability. It occurs at a rate of 3.6 cases per 1000 children, and represents a major social and psychological impact on both family and society. It is a group of disorders with movement difficulties being common for all affected patients. Its severity and extent are variable from one patient to another. Additionally, the impacts of cerebral palsy on daily activities, communications, and requirements are also variable. Recent advances in clinical research increase our knowledge and understanding of causal pathways, possible preventive measures, specific intervention strategies, and the value of new treatment modalities such as botulinum toxin and intrathecal baclofen in the management of cerebral palsy.

Abstract

In der stationären medizinischen Rehabilitation ist die kardiopulmonale Leistungsfähigkeit ein entscheidender Parameter für die sozialmedizinische Begutachtung. Als Goldstandard zur Beurteilung dieses Parameters sowie zur Bestimmung optimaler Trainingspulsbereiche gelten (spiro)ergometrische Untersuchungsverfahren, die zumeist als Tretkurbelarbeit im Sitzen oder Liegen durchgeführt werden. In der diagnostischen Praxis besitzt die Fahrradergometrie als Standardmethode zur Beurteilung des aktuellen Leistungszustandes und von Leistungsfortschritten jedoch wesentliche physiologische und ökonomische Nachteile. Vor diesem Hintergrund bieten die hier vorgestellten, walkingbasierten Ausdauertestverfahren eine sinnvolle Alternative zu den bisher in der medizinischen Rehabilitation bekannten und verwendeten Belastungsverfahren.

Abstract

"This book addresses the topic of toilet design, but instead of focusing on the typical able bodied user, it takes the various needs and limited abilities of older and/or disabled people as its starting point (human centered design). This follows the principles of ‘Inclusive Design’: designs taking into account the needs of the most challenged, will also have benefits for the healthy .For the most part, this book is a spin-off of an EU-funded research and development project called the ‘Friendly Rest Room for Elderly People’ (FFR), which ran from 2001 to 2005. During that period a consortium of 10 institutions in 7 European countries collaborated on the research, design and development of a friendlier toilet for elderly and disabled users. The contributions from partners in the FRR consortium to this book reflect the results of nearly 5 years of empirical work in different cultures, countries and disciplines. The book has been brought up-to-date with developments in this area of research in recent years.The goal of the FFR project was to carry out the necessary research and design, build and test prototypes for a Friendly Rest Room for older people and persons with a disability, enabling them to gain greater autonomy, independence, self-esteem, dignity, safety, improved self-care and thereby enjoy a better quality of life. The methods and technologies involved in fulfilling this objective included contactless smart card technologies with read-write capabilities, voice activation interface, motion control and sensor systems, mechanical engineering and robotic techniques, mathematical modelling, as well as ergonomic research and designs inspired by philosophy, gerontechnology and medical and social sciences.End-users and secondary users, as well as care takers and rehabilitation professionals, were involved in all stages of the research and problem solving processes of the FRR prototype development. Abstract

In the history of neuroscience it had long been a virtually axiomatic belief that the mature mammalian nervous system was hardwired and fixed. This view goes back to the work of Louis Broca in the 1850s and has been perhaps most famously articulated by Ramon y Cajal. The immature nervous system was thought to have considerable plasticity, but after maturity the CNS was not considered to be capable of repairing itself after damage. In the last two decades, however, persuasive evidence has been accumulating at an increasing rate that the plasticity of the nervous system persists throughout the lifespan. Beginning 14 years ago, an efficacious form of neurorehabilitation termed Constraint-Induced Movement therapy or CI therapy was shown to produce marked neuroplastic changes in the brain. It has been proposed that CI therapy harnesses neuroplasticity in the service of restoring motor and language function lost as a result of such injuries to the central nervous system as stroke, traumatic brain injury, and cerebral palsy. The proposed journal issue will include articles by the main investigators involved in the development of this body of research. There will also be articles on the role of neurogenesis in the recovery of function after CNS damage encouraged by the stimulation of endogenous stem cell production, exogenous stem cell implantation, and pharmacological means. There will also be two articles describing the work carried out to date on the use of Transcranial Magnetic Stimulation (TMS) and Transcranial Electrical Stimulation (TDCS) to increase the excitability of the brain in order to enhance the recovery of function after stroke.

Abstract

Music is an important source of enjoyment, learning, and well-being in life as well as a rich, powerful, and versatile stimulus for the brain. With the advance of modern neuroimaging techniques during the past decades, we are now beginning to understand better what goes on in the healthy brain when we hear, play, think, and feel music and how the structure and function of the brain can change as a result of musical training and expertise. For more than a century, music has also been studied in the field of neurology where the focus has mostly been on musical deficits and symptoms caused by neurological illness (e.g., amusia, musicogenic epilepsy) or on occupational diseases of professional musicians (e.g., focal dystonia, hearing loss). Recently, however, there has been increasing interest and progress also in adopting music as a therapeutic tool in neurological rehabilitation, and many novel music-based rehabilitation methods have been developed to facilitate motor, cognitive, emotional, and social functioning of infants, children and adults suffering from a debilitating neurological illness or disorder. Traditionally, the fields of music neuroscience and music therapy have progressed rather independently, but they are now beginning to integrate and merge in clinical neurology, providing novel and important information about how music is processed in the damaged or abnormal brain, how structural and functional recovery of the brain can be enhanced by music-based rehabilitation methods, and what neural mechanisms underlie the therapeutic effects of music. Ideally, this information can be used to better understand how and why music works in rehabilitation and to develop more effective music-based applications that can be targeted and tailored towards individual rehabilitation needs. The aim of this Research Topic is to bring together research across multiple disciplines with a special focus on music, brain, and neurological rehabilitation. We encourage researchers working in the field to submit a paper presenting either original empirical research, novel theoretical or conceptual perspectives, a review, or methodological advances related to following two core topics: 1) how are musical skills and attributes (e.g., perceiving music, experiencing music emotionally, playing or singing) affected by a developmental or acquired neurological illness or disorder (for example, stroke, aphasia, brain injury, Alzheimer’s disease, Parkinson’s disease, autism, ADHD, dyslexia, focal dystonia, or tinnitus) and 2) what is the applicability, effectiveness, and mechanisms of music-based rehabilitation methods for persons with a neurological illness or disorder? Research methodology can include behavioural, physiological and/or neuroimaging techniques, and studies can be either clinical group studies or case studies (studies of healthy subjects are applicable only if their findings have clear clinical implications).

Abstract

Health and well-being is best understood in terms of a combination of biological, psychological, and social factors. But how ‘social’ is the biopsychosocial model when applied to mental health and rehabilitation? Psychology has traditionally viewed health as being determined by individual behavior. An integrative psychological approach is required to draw understanding from sociology, social psychology, and politics to consider how wider systemic, structural, and contextual factors impact on health behavior and outcomes. Abstract

Rehabilitation of cognitive functions is a primary goal in neurological and psychiatric settings. Cognitive treatments include individual and group exercises, as well as the use of computer programs and virtual reality. Besides, ongoing studies have been examining the clinical usefulness of non-invasive cerebral cortex stimulation in increasing the efficacy of cognitive protocols. Cognitive rehabilitation is based on neuroplasticity and affects brain morphological and physiological responses by integration of behavioral and cognitive changes. The brain correlates of rehabilitation-induced modifications can be investigated through magnetic resonance imaging, both at structural and functional macro-levels. Animal research can integrate such information providing data on axonal regrowth and reshaping of synaptic connectivity in response to treatment. Animal models of neurological and psychiatric conditions have been developed, and preclinical test batteries for the assessment of cognitive functions in animal models of such conditions have been created. The question is then: how does rehabilitation drive reorganization at the neuronal level? The focus of this Research Topic is on rehabilitation-induced cognitive and neural plasticity in adult humans and animal models. The goal is to provide an integrated picture highlighting what techniques have been proven to be effective.
