Abstract

​This work is devoted to understanding the recent advances in nuclear medicine and molecular imaging technologies along with their application to integrated medical therapy and future drug development. This anthology is based on the international symposium in 2015 entitled “Perspective on Nuclear Medicine for Molecular Diagnosis and Integrated Therapy. “The symposium provided an opportunity to exchange ideas on how to promote nuclear medicine technology and how to extend the technology to medical therapy and drug development, and was also a good opportunity to discuss the future perspective of nuclear medicine and molecular imaging by worldwide leaders in the field. Molecular imaging technologies have been rapidly developed worldwide in recent years. Among those developments, nuclear medicine has come to play an important role in quantitative analysis of biological process in vivo as well as in wide clinical use. With the current progress of nuclear medicine and molecular imaging, this modality has been applied for treatment monitoring and predicting its outcome with the use of optimal imaging biomarkers and suitable quantitative analysis. Truly, a new era has arrived with clinical use of nuclear medicine and molecular imaging for personalized medicine.This volume will benefit a wide variety of researchers in life science including those working in drug development, molecular imaging, and medical therapy as well as physicians who utilize diagnostic imaging.

Abstract

This volume describes and explains the educational method of Case-Based Clinical Reasoning (CBCR) used successfully in medical schools to prepare students to think like doctors before they enter the clinical arena and become engaged in patient care. Although this approach poses the paradoxical problem of a lack of clinical experience that is so essential for building proficiency in clinical reasoning, CBCR is built on the premise that solving clinical problems involves the ability to reason about disease processes. This requires knowledge of anatomy and the working and pathology of organ systems, as well as the ability to regard patient problems as patterns and compare them with instances of illness scripts of patients the clinician has seen in the past and stored in memory. CBCR stimulates the development of early, rudimentary illness scripts through elaboration and systematic discussion of the courses of action from the initial presentation of the patient to the final steps of clinical management.The book combines general backgrounds of clinical reasoning education and assessment with a detailed elaboration of the CBCR method for application in any medical curriculum, either as a mandatory or as an elective course. It consists of three parts: a general introduction to clinical reasoning education, application of the CBCR method, and cases that can used by educators to try out this method.

Abstract

The idea of combining drugs and diagnostics in oncology is not new. When the selective estrogen receptor modulator tamoxifen was developed in the 1970’s for the treatment of breast cancer a positive correlation between receptor status and treatment outcome was found. As a result of this research, it was suggested to use the estrogen-receptor assay as a diagnostic test for selection of patients for tamoxifen treatment. Despite this suggestion was put forward nearly 40 years ago the adaptation of the drug-diagnostic co-development model has been relatively slow and it is only within the last decade that it has gained more widespread acceptance. The parallel development of the monoclonal antibody trastuzumab (Herceptin®, Roche/Genentech) and the immunohistochemistry assay for HER2 protein overexpression (HercepTest™, Dako) seems to have served as an inspiration to a number of stakeholders such as pharma and diagnostic companies, regulatory agencies, and academia. In recent years we have seen an increasing number of oncology drug development projects that have taken advantage of the drug-diagnostic co-development model, as outline below. Most of the new targeted anti-cancer drugs that have been introduced in recent years, such as BRAF-, ALK-, EGFR- and HER2-inhibitors, are more or less all a product of the drugdiagnostic co-development model. These drugs have shown remarkable high response rates in selected groups of patients within cancer diseases with great unmet medical needs. Abstract

La récente émergence du SRAS (syndrome respiratoire aigu sévère) rappelle la vulnérabilité de l’homme face aux maladies infectieuses. La diffusion possible de certains virus à l’échelle planétaire, liée au développement des transports et à leur rapidité croissante, fait que nous sommes désormais tous concernés. À travers de nombreux exemples, cet ouvrage présente le concept de virus émergents et analyse les facteurs qui favorisent cette émergence. L’homme y tient une place déterminante par les modifications majeures qu’il apporte à l’écosystème et par l’évolution très rapide de ses modes de vie (urbanisation, politiques de santé, pratiques socio-culturelles). Sont également exposées les propriétés évolutives des virus – en particulier des virus à ARN – au niveau biologique et moléculaire, qui jouent un rôle important dans leur adaptation à l’homme et leur diffusion. Quelles menaces pour le futur ? L’un des risques majeurs réside dans l’extrême diversité des virus découverts dans les zones tropicales, virus potentiellement responsables des maladies émergentes de demain. Seule l’implantation durable de centres de recherche et de surveillance dans ces zones peut permettre la détection précoce de ces nouveaux agents. L’expansion des viroses existantes est également analysée, ainsi que les risques entraînés par l’apparition de virus mutants, susceptibles de provoquer une nouvelle pandémie de grippe.

Abstract

Single-domain antibodies (sdAbs) represent the minimal antigen binding-competent form of the immunoglobulin domain and have unique properties and applications. SdAbs are naturally produced as the variable domains of the heavy chain-only antibodies of camelid ruminants and cartilaginous fishes, but can also be engineered synthetically from autonomous human or mouse VH or VL domains. The scope of this research topic and associated e-book covers current understanding and new developments in (i) the biology, immunology and immunogenetics of sdAbs in camelids and cartilaginous fishes, (ii) strategies for sdAb discovery, (iii) protein engineering approaches to increase the solubility, stability and antigen-binding affinity of sdAbs and (iv) specialized applications of sdAbs in areas such diagnostics, imaging and therapeutics.

Abstract

Gastric cancer is a leading cause of cancer deaths, but analysis of its molecular and clinical characteristics has been complicated by histological and aetiological heterogeneity. Stomach cancers are 90% adenocarcinoma; lymphoma, carcinoid, and stromal tumours may occur. Adenocarcinoma can be subdivided into histological Lauren and the World Health Organization (WHO) classifications, but this information has not led to the development of histologic subtype-specific treatment options. One way to potentially improve treatment for gastric cancers is to better understand the pathogenesis of this disease, the contribution of Helicobacter pylori infection, and host immune response to lead to the development of integrated histological and molecular classification schemes for gastric cancer. The hope is that these studies may facilitate the development of clinical trials to explore therapies in defined sets of patients, ultimately improving survival from this deadly disease.

Abstract

Le trachome, infection oculaire pouvant entraîner la cécité, touche plus de 80 millions d’individus dans le monde. Encore présent avant la Première Guerre mondiale dans les pays européens, il en a disparu grâce au développement socio-économique. Sa persistance dans les régions les plus pauvres du globe peut être considérée comme un stigmate de la pauvreté. Pourtant cette maladie, première cause de cécité d’origine infectieuse dans le monde, peut être prévenue et traitée à l’aide d’un certain nombre de mesures de santé publique. Des actions d’envergure ont ainsi été entreprises depuis une dizaine d’années au niveau mondial sous l’égide de l’Organisation mondiale de la santé, avec l’ambition d’éliminer le trachome cécitant d’ici l’an 2020. Les mesures médicales et chirurgicales mises en œuvre sont associées à des campagnes éducatives et sanitaires visant à améliorer l’hygiène et l’environnement. Cet ouvrage retrace l’histoire du trachome et offre une synthèse des connaissances actuelles. Il fait le point sur les déterminants biologiques et sociaux de cette maladie complexe, expose les différentes composantes de la stratégie de lutte et présente les données épidémiologiques les plus récentes concernant l’Afrique subsaharienne, région du monde particulièrement touchée. Abstract

Dielectric liquids are used as impregnates of solid insulations or filling products in many kinds of equipment, such as transformers (power, rectifier, distribution, traction, furnace, potential, current), resistors, reactors, capacitors, cables, bushings, circuit breakers, tap changers, and thyristor cooling in power electronics, etc. Their role is paramount in the sense that their presence is one of the fundamental conditions for the proper functioning of such equipment. Similar to blood in the human body, dielectric liquids hold the responsibility of maintaining the entire equipment. The greatest challenges for engineers/maintenance planners are related to the need for methods to assess the condition of the liquid, along with how to improve equipment efficiency using chemical admixtures. The demand for environmentally friendly dielectric liquids is also rising as the environmental concerns about conventional petroleum-based fluids become increasingly more apparent.This book covers some theoretical and practical concerns related to the applications of dielectric liquids in electrical equipment.
