Abstract

This chapter will take as an example a particular change in healthcare financing at the meso-level: Diagnosis-Related Groups. DRGs marked a shift in the way hospitals were oriented and were the means to operationalise the market-logic in healthcare. The DRG system is a case classification system: each hospital case is related to a ‘pre-standardised product’ of treatment. Depending on how a patient’s case is encoded, the hospital is reimbursed for the services provided. In this way, hospital care can be matched with pre-calculated resources. Case volumes can be predetermined according to the profit needs of a hospital and an economy’s ‘healthcare sector’ as a whole. Developed in the early 1980s by Robert B. Fetter and John D. Thompson, a management and a public health scholar at Yale, DRGs were first introduced in New Jersey, and have become the key accounting system throughout Western and increasingly LMI countries’ healthcare systems. (Mathauer and Wittenbecher 2013) Here I will particularly draw on examples from Germany, Switzerland and Scandinavia, but also to the NHS in England

Abstract

On the one hand, sport like any other activity in society is burdened with public taxes, and on the other hand, in comparison to similar activities, is tax-free and even publicly supported. This ambivalence of tax treatment of sport has repeatedly raised questions about the public financing of sport: To what extent are services provided by the public sector justified, and to what extent do sporting events trigger taxation? This volume, edited by Hans-Peter Büch, Wolfgang Maennig and Hans-Jürgen Schulke, attempts to answer selected questions on this problem from a scientific and sports-political point of view under the motto "Science meets Practice".

Abstract

The financial crisis has opened up a global debate on the taxation of the financial sector. A number of international policy initiatives, most notably by the G20, have called for major changes in the tax treatment of financial institutions and transactions as well as individuals working in the financial sector. This book examines how tax policies contributed to the financial crisis and whether taxation can play a role in the reform efforts under way to establish a sounder and safer financial system. The book looks at the pros and cons of various tax initiatives, including limiting the tax advantages to debt financing, special taxes on the financial sector and financial transactions taxes.

Abstract

Developing countries interested in REDD+ under the UNFCCC have been requested to prepare a national forest monitoring system (NFMS) and a system to monitor, report and verify implementation (MRV). They have also been requested to engage local communities and indigenous groups as critical stakeholders in this process. The NFMS should be consistent with national inventories of emissions and removals of greenhouse gases. These inventories are, however, usually prepared using national level information with low geographical resolution and without the participation of local communities or other forest owners/managers. However, it has been shown that members of rural forest communities can develop the skills to monitor and measure levels of carbon stock in their forests and changes in these levels over time. If this information could be included and tracked from the local to regional and national levels, this might help to design transparent mechanisms for the assessment of REDD+ implementation, and possibly even for benefit sharing. The objective of this Special Issue is to discuss and explore the social, technical and political implications and potential of including community-based monitoring in MRV systems and benefit-sharing schemes in REDD+.

Abstract

During the recent few decades, global economic growth has been driven largely by developing world economies. The ones with the most intensive pace of development were marked as “emerging“ markets led by so called BRICS and N-11 countries. Such changes inevitably reflected the global health arena. A number of issues previously limited to established high-income economies became popularly discussed topics on the agendas of public health policy makers across these regions. Major challenges remain population aging, rising incidence of prosperity diseases, lack of universal insurance coverage and particularly provision of just and equitable access to medical care among the poor both in urban and rural communities. A significant part of the difficulties faced by these societies are attributed to inefficient resource allocation strategies in health care and unsatisfactory funding strategies. This Research Topic was created in order to address the core challenges of medical care financing and its affordability across the emerging global markets. Contributions of both undergoing or finished original research as well as review style papers are welcomed. All submitted manuscripts should deal with issues relevant to health care economics and policy in recognized global emerging markets. Outside the aforementioned key markets (BRICS- Brazil, Russia, India, China, South Africa; Next 11- Bangladesh, Egypt, Indonesia, Iran, South Korea, Mexico, Nigeria, Pakistan, the Philippines, Turkey and Vietnam) submissions referring to any of the dynamically developing Asian, Latin America, Eastern Europe or MENA countries are encouraged. In addition to a variety of health-economic evaluations and health policy analysis, methodological and resource use studies are within the Topic scope. Health policy considerations should be primarily focused on financing mechanisms and affordability of health care although other surrounding issues such as health insurance, reimbursement and cost-containment strategies will be considered.

Abstract

The authors of this book advise the economies of the European Union to become more entrepreneurial in promoting innovation and economic growth. The authors propose a reform strategy with respect to several aspects to achieve this goal. Starting with the rule of law and the protection of property rights; the tax system; the authors deal with regulations governing savings, capital and finance, and the organization of labor markets and social insurance systems. Framework strategies related to the regulations governing goods and service markets, bankruptcy and insolvency are also put forward. A core understanding and future path is also provided towards R&D, commercialization and knowledge spillovers; human capital investments; and informal institutions.

Abstract

Apart from financing retirement provision, assistance and care for elderly people ist he central issue in Swiss politics concerning elderly people. While medical care has been in focus of the discourse about health and social politics for a while, the issue of care work was often only touched upon. This book discusses legal issues concerning medical care and care work, describes the involved actors, the education, the recruitment and the wages of employees. Furthermore, self-sustained and health promoting living at a high age is an issue. It becomes evident that care work is neither legally defined nor with regards to content. Legal regulations securing quality standards, legal stability for employees as well as the integration of care work into the system of social security, are lacking in practice. In view of these conditions and changes and society, this book shows that the issue of care work needs more consideration in the political discourse concerning elderly people.&#xD;

Abstract

This book focuses on the fiscal decentralisation processes and the systems of local government financing in Serbia and Montenegro in the period 2000-2015. The retrospective analysis of the decentralisation processes in both countries show that they have been moving back and forth between decentralisation and centralisation, constantly swinging the pendulum of political discourse and the legal framework on intergovernmental fiscal governance. During the observed period, the systems of local government financing in both countries have often undergone drastic changes. Thus, this study focuses on the analysis of the impact of the legal framework related to intergovernmental fiscal relations, fiscal decentralisation and local government financing in Serbia and Montenegro on local government budgets. By applying both normative and economic analyses, as well as both quantitative and qualitative research methods, the study evaluates the legal quality and economic and factual effects of relevant legislation on local government budgets and status in the period 2000-2015. The study is divided into four parts: 1) Introduction; 2) Case Study: Serbia; 3) Case Study: Montenegro (both case studies include normative, economic and empirical analyses of the fiscal decentralisation process and the system of local government financing); 4) Comparative conclusions and recommendations.

Abstract

While mental health care has undergone substantial reforms, little attention has been paid to financing issues. This book addresses this shortcoming and brings more transparency into the complex relationship between mental health care reform, service provision and financing. Additionally, it provides rich information about the characteristics of mental health care financing in Western Europe. The author analyses the recent mental health care reform discourse against the backdrop of broader political economic developments and demonstrates the role of financing arrangements herein. The book vividly shows how financing is related to specific effects for service users and their relatives in the process of change. In the final part practitioners, planers and policy makers find useful guidelines for developing alternative financing approaches including support to improve understanding of financing issues amongst those involved in mental health care.
