Abstract

The authors of this book set out a system of safety strategies and interventions for managing patient safety on a day-to-day basis and improving safety over the long term. These strategies are applicable at all levels of the healthcare system from the frontline to the regulation and governance of the system.There have been many advances in patient safety, but we now need a new and broader vision that encompasses care throughout the patient’s journey. The authors argue that we need to see safety through the patient’s eyes, to consider how safety is managed in different contexts and to develop a wider strategic and practical vision in which patient safety is recast as the management of risk over time. Most safety improvement strategies aim to improve reliability and move closer toward optimal care. However, healthcare will always be under pressure and we also require ways of managing safety when conditions are difficult. We need to make more use of strategies concerned with detecting, controlling, managing and responding to risk. Strategies for managing safety in highly standardised and controlled environments are necessarily different from those in which clinicians constantly have to adapt and respond to changing circumstances.This work is supported by the Health Foundation. The Health Foundation is an independent charity committed to bringing about better health and health care for people in the UK.The charity’s aim is a healthier population in the UK, supported by high quality health care that can be equitably accessed. The Foundation carries out policy analysis and makes grants to front-line teams to try ideas in practice and supports research into what works to make people’s lives healthier and improve the health care system, with a particular emphasis on how to make successful change happen.A key part of the work is to make links between the knowledge of those working to deliver health and health care with research evidence and analysis. The aspiration is to create a virtuous circle, using what works on the ground to inform effective policymaking and vice versa. Good health and health care are vital for a flourishing society. Abstract

This book provides standards and guidelines for quantifying greenhouse gas emissions and removals in smallholder agricultural systems and comparing options for climate change mitigation based on emission reductions and livelihood trade-offs. Globally, agriculture is directly responsible for about 11% of annual greenhouse gas (GHG) emissions and induces an additional 17% through land use change, mostly in developing countries. Farms in the developing countries of sub-Saharan Africa and Asia are predominately managed by smallholders, with 80% of land holdings smaller than ten hectares. However, little to no information exists on greenhouse gas emissions and mitigation potentials in smallholder agriculture. Greenhouse gas measurements in agriculture are expensive, time consuming, and error prone, challenges only exacerbated by the heterogeneity of smallholder systems and landscapes. Concerns over methodological rigor, measurement costs, and the diversity of approaches, coupled with the demand for robust information suggest it is germane for the scientific community to establish standards of measurements for quantifying GHG emissions from smallholder agriculture. Standard guidelines for use by scientists, development organizations will help generate reliable data on emissions baselines and allow rigorous comparisons of mitigation options. The guidelines described in this book, developed by the CGIAR Research Program on Climate Change, Agriculture, and Food Security (CCAFS) and partners, are intended to inform anyone conducting field measurements of agricultural greenhouse gas sources and sinks, especially to develop IPCC Tier 2 emission factors or to compare mitigation options in smallholder systems.

Abstract

This book summarizes work being pursued in the context of the CIPRNet (Critical Infrastructure Preparedness and Resilience Research Network) research project, co-funded by the European Union under the Seventh Framework Programme (FP7). The project is intended to provide concrete and on-going support to the Critical Infrastructure Protection (CIP) research communities, enhancing their preparedness for CI-related emergencies, while also providing expertise and technologies for other stakeholders to promote their understanding and mitigation of the consequences of CI disruptions, leading to enhanced resilience. The book collects the tutorial material developed by the authors for several courses on the modelling, simulation and analysis of CIs, representing extensive and integrated CIP expertise. It will help CI stakeholders, CI operators and civil protection authorities understand the complex system of CIs, and help them adapt to these changes and threats in order to be as prepared as possible for mitigating emergencies and crises affecting or arising from CIs.

Abstract

By means of this ‘Frontiers in Genetics’ research topic, we are celebrating 30 years of the Comet Assay. The first paper on this single-cell gel electrophoresis assay was published in 1984 by O. Ostling and K.J. Johanson (Biochem. Biophys. Res. Commun. Vol.123: 291-298). The comet assay is a versatile and sensitive method for measuring single - and double-strand breaks in DNA. By including lesion-specific enzymes in the assay, its range and sensitivity are greatly increased, but it is important to bear in mind that their specificity is not absolute. The comet assay (with and without inclusion of lesion-specific enzymes) is widely used as a biomarker assay in human population studies - primarily to measure DNA damage, but increasingly also to assess the capacity of cells for DNA repair. Ostling and Johanson (Biochem. Biophys. Res. Commun., 1984) were also the first to report experiments to measure DNA repair, by simply following the decrease of DNA damage over time after challenging cells with ionising radiation. However, this approach is time-consuming and laborious as it requires an extended period of cell culture and is therefore not ideal for biomonitoring studies, which typically require high-throughput processing of many samples. As an alternative approach, the in vitro comet-based repair assay was developed: a cell extract is incubated with a DNA substrate containing specific lesions, and DNA incisions accumulate. The in vitro comet-based repair assay has been modified and improved over the past decade: it was first devised to measure base excision repair of oxidised purines in lymphocytes (Collins et al., Mutagenesis, 2001), but has since been adapted for other lesions and thus other repair pathways, as well as being applied to tissue samples in addition to cell suspensions. Even after 30 years, the comet assay is still in a growth phase, with many new users each year. Many questions are repeatedly raised, which may seem to have self-evident answers, but clearly, it is necessary to reiterate them for the benefit of the new audience, and sometimes being forced to think again about old topics can shed new light. Different applications of the comet assay are discussed in this special issue, including: genotoxicity testing in different organisms, human biomonitoring, DNA repair studies, environmental biomonitoring and clinical studies. Furthermore, we consider and where possible answer questions, including the ones raised by Raymond Tice at the 8th International Comet Assay Workshop in Perugia (Italy 2009): What is the spectrum of DNA damage detected by the various versions of the comet assay?; What are the limitations associated with each application?; What should be done to standardize the assay for biomonitoring studies?; Can the comet assay be used to monitor changes in global methylation status?; What cell types are suitable for detecting genotoxic substances and their effects in vivo and in vitro?; Can the assay be fully automated?; and more. So this ‘Frontiers in Genetics’ research topic is written for the beginner as well as for the experienced users of the comet assay.

Abstract

The continuous miniaturization of products and the growing complexity of their embedded multifunctionalities necessitates continuous research and development efforts regarding micro components and related micro manufacturing technologies. Highly miniaturized systems, manufactured using a wide variety of materials, have found application in key technological fields, such as healthcare devices, micro implants, mobility, communications, optics, and micro electromechanical systems. Innovations required for the high-precision manufacturing of micro components can specifically be achieved through optimizations using post-process (i.e., offline) and in-process (i.e., online) metrology of both process input and output parameters, as well as geometrical features of the produced micro parts. However, it is of critical importance to reduce the metrology and optimization efforts, since process and product quality control can represent a significant portion of the total production time in micro manufacturing. To solve this fundamental challenge, research efforts have been undertaken in order to define, investigate, implement, and validate the so-called “product/process manufacturing fingerprint” concept. The “product manufacturing fingerprint” concept refers to those unique dimensional outcomes (e.g., surface topography, form error, critical dimensions, etc.) on the produced component that, if kept under control and within specifications, ensure that the entire micro component complies to its specifications. The “process manufacturing fingerprint” is a specific process parameter or feature to be monitored and controlled, in order to maintain the manufacture of products within the specified tolerances. By integrating both product and process manufacturing fingerprint concepts, the metrology and optimization efforts are highly reduced. Therefore, the quality of the micro products increases, with an obvious improvement in production yield. Accordingly, this Special Issue seeks to showcase research papers, short communications, and review articles that focus on novel methodological developments and applications in micro- and sub-micro-scale manufacturing, process monitoring and control, as well as micro and sub-micro product quality assurance. Focus will be on micro manufacturing process chains and their micro product/process fingerprint, towards full process optimization and zero-defect micro manufacturing.
