Abstract

In 2003-2006, Patricia Henderson lived in the South African province of KwaZulu-Natal where she recorded the experiences of people living with HIV/AIDS. In this illuminating study, she recounts the concerns of rural people and explores local repertoires through which illness was folded into everyday life. The book spans a period when antiretroviral medication was not available, and moves on to a time when the treatment became accessible. Hope gradually became manifest in the recovery of a number of people through antiretroviral therapies and 'the return' of bodies they could recognise as their own. This research implies that protracted interaction with people over time, offers insights into the unfolding textures of everyday life, in particular in its focus on suffering, social and structural inequality, illness, violence, mourning, sensibility, care and intimacy.

Abstract

This book is an open access dissemination of the EU COST Action ADMIRE in Aldosterone/Mineralocorticoid Receptor (MR) physiology and pathophysiology. Aldosterone is the major hormone regulating blood pressure. Alterations in blood levels of aldosterone and genetic mutations in the MR receptor are major causes of hypertension and comorbidities. Many of the drugs in clinical use, and in development for treating hypertension, target aldosterone and MR actions in the kidney and cardiovascular system. The ADMIRE book assembles review chapters from 16 European ADMIRE laboratories providing the latest insights into mechanisms of aldosterone synthesis/secretion, aldosterone/MR physiology and signaling, and the pathophysiological roles of aldosterone/MR activation.

El orden del día de las agendas de los estudiosos de la alimentación está repleto de fenómenos que, formando parte de procesos históricos más amplios, deben ser tratados desde el conocimiento de las diferencias culturales, de las diversas formas de desigualdad social y de las implicaciones sociales, económicas y políticas que la comida tiene para la salud humana, el entorno y las sociedades. La producción, distribución y consumo de alimentos implica numerosos ámbitos y agentes en cualquier lugar y nos llevan desde las actividades agrícolas hasta el procesamiento de los alimentos, desde el restaurante o el comedor escolar al hogar, desde el individuo al grupo social; intervienen, a su vez, en el proceso de salud/enfermedad/atención. A pesar de la abundancia aparente, el sistema de producción alimentaria actual no asegura las necesidades básicas entre las personas, ni el reparto equitativo de los alimentos, ni la capacidad regenerativa de los recursos utilizados ni tampoco la preservación de la diversidad biocultural. Tampoco favorece la confianza en los alimentos producidos, ni el deseo tan humano como legítimo de querer preservar y mejorar la calidad de vida.

Anopheles mosquitoes are highly important insects due to their involvement in the transmission of human malaria and its devastating consequences in endemic countries worldwide. In 2010 alone, malaria was responsible for an estimated 660,000 deaths. As the study of Anopheles species and populations is a key element for reaching the goal of malaria elimination, an enormous amount of information has accumulated over the past century, and together in recent decades with the advent of novel technologies the acquisition of new knowledge has accelerated even further. Abstract

Not only developed countries, but also most developing areas of the world, have experienced a surge in obesity prevalence over recent decades. Obesity complications are now among the leading causes of premature mortality, encompassing conditions such as coronary heart disease, stroke, and type 2 diabetes. This places a heavy burden on contemporary healthcare systems. While rodent models have limitations as experimental models of human obesity-related disease, study of rats and mice either spontaneously prone - or resistant - to obesity, or genetically engineered to illuminate underlying mechanisms has yielded key information about the metabolic defects linked to obesity, and their associated diseases. This topic includes both original research studies and reviews of the use of animal studies in specific areas of obesity-related disease. Various methodological approaches are discussed, with evaluation of the extent to which use of animal models has facilitated progress, or, conversely, has proved a cul de sac in investigation of human disease mechanisms. Consideration is also given to future strategies to use such rodent models optimally to enhance comprehension and treatment of pandemic human obesity-related diseases.

Abstract

There is extensive evidence from animal models that gonadal steroids, produced in fetal and neonatal life, act on the developing organism to produce sex differences far beyond the reproductive system. That early gonadal steroid exposure also plays an important role in human development is supported by studies of individuals with disorders of sex determination and differentiation. It is much less clear whether normal variation in gonadal steroid exposure predicts sexually dimorphic health outcomes or within-sex variation. This is largely due to challenges related to the assessment of gonadal steroid exposure in the developing fetus and neonate. Regarding the prenatal period, serial measurements of serum hormone levels in the fetus, for use in studies of later development, are not possible for ethical reasons. Researchers have measured hormones in maternal blood, umbilical cord blood, and amniotic fluid; used putative anthropometric indices such as the relative lengths of the 2nd and 4th digits (2D:4D); evaluated common variants in genes related to hormone production, transport, and metabolism; and examined development in opposite sex twins and the offspring of mothers with hyperandrogeny. Each of these approaches has particular strengths and notable weaknesses. Regarding the neonatal period, serial measurements in serum are often impractical for studies of typical development. Salivary hormone assays, frequently used in studies of older children and adults, have not been extensively investigated in neonates. The most appropriate timing for testing is also open to debate. Early work suggested that testosterone levels in males begin to rise after the first postnatal week, peak around the 3rd to 4th months of life, and then drop back to very low levels by 1 year. However a more recent study of 138 infants did not demonstrate this pattern. Testosterone was highest on the day of birth and gradually dropped over the first 6 months. Even less is known about patterns of early estrogen exposure, though highly sensitive bioassays indicated that sex differences are present in early childhood. In addition, the design and interpretation of studies may be impacted by widespread acceptance of conceptual frameworks that are not well-supported empirically. For example, many researchers presume that the free hormone hypothesis, which states that unbound hormone is more readily diffusible into tissues and thus a better measure of actual exposure, is true. However this hypothesis has been challenged on multiple grounds. A second example: it is generally accepted that masculinization of the human brain is primarily mediated by the androgen receptor (in contrast to rodents where the estrogen receptor plays a major role), in part because chromosomal males with complete androgen insensitivity generally espouse a female gender identity. However this is not always the case, and other sexually dimorphic outcomes have not been carefully assessed in CAIS. The aim of this research topic is to gather together experimental and review papers which address the diverse challenges in assessing prenatal and neonatal gonadal steroid exposure for studies of human development with the expectation that this will allow more critical appraisal of existing studies, identify critical research gaps, and improve the design of future studies.

Abstract

Autoimmune thyroid diseases (AITD) are T cell-mediated organ-specific autoimmune disorders resulting from an immune dysregulation leading to a thyroid immune attack (Antonelli and Benvenga). Graves’ disease and Hashimoto’s thyroiditis are the two main clinical presentations of AITD, and their clinical hallmarks are thyrotoxicosis and hypothyroidism, respectively. In many cases, AITD may be associated in the same patient with other organ-specific autoimmune attacks (such as in the case of type II autoimmune polyglandular syndrome, or type I diabetes, etc). Furthermore, AITD and thyroid function abnormalities have been frequently described in patients with systemic rheumatologic autoimmune diseases. Conversely, patients affected with the above mentioned autoimmune disorders are more frequently affected by AITD. In this Research Topic, constituted by nineteen papers, we review and discuss new evidence about the association of other autoimmune diseases in patients with AITD. Among other organ-specific autoimmune disorders, the associations of AITD with chronic autoimmune gastritis (Cellini et al.), vitiligo (Baldini E et al.), lichen (Guarneri et al.), psoriasis (Ruffilli et al.), myasthenia gravis (Lopomo and Berrih-Aknin) and glomerulopathies (Santoro et al.) have been treated. Also the associations of AITD, in systemic autoimmune diseases have been treated (as Sjögren’s syndrome, Baldini C et al.; systemic sclerosis, Fallahi et al.; systemic lupus erythematosus, Ferrari et al.; Antiphospholipid syndrome, Versini; sarcoidosis, Fazzi et al.; the autoimmune/inflammatory syndrome induced by adjuvants, Watad et al.; rheumatoid arthritis, Bliddal et al.; Hepatitis C Virus and mixed cryoglobulinemia, Ferri et al.; and, psoriathic arthritis, Ruffilli et al.). Furthermore peculiar aspects associated with post partum thyroiditis have been reviewed too (Di Bari et al., Le Donne et al.). The exact pathogenetic mechanisms underlying the above reported associations are not completely known. It has been hypothesized that the influence of genetic (Coppedè), and environmental factors (Antonelli and Benvenga), could lead to the onset of autoimmune phenomena in different organs in the same subject, characterized by predominance of a Th1 immune pattern at the beginning, and in the active phase of these disorders. In conclusion, an association of other autoimmune diseases in patients with thyroid autoimmunity has been shown, and this Research Topic provides an extensive update of the literature, and suggests interesting points for new investigations.
